What You Need To Know About Steroid Injections

An H&L injection is a combination of a steroid and a local anaesthetic.

H&L is used for its anti-inflammatory properties and also relieves pain at the target site.

Historically, the steroid used was hydrocortisone "H". The local anaesthetic component is lignocaine "L". These days, we use steroids such as betamethasone or triamcinolone that are more potent than hydrocortisone.

H&L injections may be used to treat musculoskeletal conditions such as trigger finger or tennis elbow.

In aesthetics, they are mainly used for acute acne flares (1-5 pimples) and keloids.

How it works:

H&L is injected in very small doses (<0.05ml) directly into the pimple. The steroid component rapidly brings down the pain and inflammation. Within 24 hours, the pimple will be smaller, flatter and less painful.

Who is suitable for this treatment?

Anyone with an acute flare of acne can be suitable for H&L injection.

What are the risks or possible side effects?

Repeated H&L injections can cause thinning of the skin or hypopigmentation.

When should I go for this treatment?

If there is a severe acne flare with multiple, large pimples, you may consider H&L injection to reduce the risk of scarring. It can also be used to speed up recovery.

How it works:

Keloids are abnormal scars that forms at the site of injury e.g. cuts, burns, abrasions. The steroid breaks down collagen within the scar, making it softer and lighter in colour. It also reduces swelling and itchiness.

Who is suitable for this treatment?

Anyone with an acute flare of acne can be suitable for H&L injection.

What are the risks or possible side effects?

H&L injections can cause thinning of the skin or hypopigmentation. Sometimes, keloids may become flat or even indented, and white. This is associated with higher volume injections per session and multiple sessions (>10).

When should I go for this treatment?

If you have keloids that are cosmetically concerning, symptomatic (itchy and painful) or restricting movement, you should consider H&L injections.
